ONLINE FORM INSTRUCTIONS:

ATTENTION: PDF must be downloaded and opened with the latest version of Adobe Reader on all devices. Just clicking on the Employment Application link may open it within your browser and you may not be able to submit it. We recommend using a computer for the best experience and results.

  1. Download the latest version of Adobe Reader for Computer, Android, iPhone & iPad.

  2. Computer: Right click on the form link and choose "Save Link As" from the drop down menu. Save it to your computer or device.

  3. If you MUST use an iPhone or iPad: Tap the application link above. When it opens tap the document and tap "Open in". Tap "Open in Adobe Acrobat".

  4. Android/Galaxy phones: Tap the application link above. Adobe Reader should open if it is installed.

  5. Fill out the application.

  6. Click the "Email Form" at the bottom of the form. Attach any relative files to the email.

  7. Print it and sign it and mail it with copies of any relative or current certifications to:

  8. Or drop the completed form by our main office at the 69 N. Paradise Parkway, Bldg B. in Garden City Utah between 9:00 am - 5:00 pm, Monday - Friday.
